For Seafarers Working on Cruise Ships, Ferries, and Yachts
Ensuring the health and safety of all crew members at sea is paramount. A Pre-Employment Medical Examination (PEME) is required by many companies to help to certify that you are fit for duty aboard a vessel. Cruise ships and Super Yachts require these prior to employment to ensure your fitness to work in specific areas aboard a ship and to help reduce any health issues whilst employed.

Why do I need a PEME medical aswell as an ENG1 medical ?
A PEME medical is a pre-employment medical examination (PEME) required by certain Cruise ships and Super Yachts to help ensure their staff are fit for specific duties as well as helping to provide assurance that their employees will remain fit for duty whilst working. An ENG1 medical is also required prior to working at sea but this is a mandatory medical required by the MCA ( link) to ensure that the seafarer is fit to work aboard the ship prior to embarkment as well as periodically up to Two yearly
Can I pass a PEME medical but fail an ENG1 medical ?
The requirements are similar and this would not be likely to occur
Can I have a PEME medical at the same time as an ENG1 medical?
Yes, we can perform these medicals at the same time. Please contact us for further information about costing of combining these medicals.
If I require further tests for my PEME medical can they be performed at Occupational Health Bristol ?
Additional tests such as Chester or Queens fitness tests, drug and alcohol testing, blood tests, Keystone vision testing, ECGs and Spirometry ( formal lung function testing ) can occur at Occupational Health bristol. We arrange Chest Xray and Ultrasounds scans at a local clinic if required. The results are typically back within 7 working
What is involved in the Pre-employment medical examination?
The basic medical is similar to an ENG1 medical. It is designed to assess your overall fitness for working at sea. It typically includes:
A detailed review of your medical history.
A complete physical examination, including measurement of height, weight, blood pressure, and pulse.
Vision Testing: Assessment of your distance, near, and colour vision (using an Ishihara test).

Hearing Test: An audiometry assessment to check your hearing levels.

Urinalysis: A urine sample is tested for markers such as protein, blood, and sugar.

Further tests, such as blood tests, ECG, spirometry or a chest X-ray, may be required depending on the specific flag state or cruise line requirements.

Can I pass a PEME and or ENG1 with Pre-existing Conditions?
Having a pre-existing medical condition does not automatically prevent you from working at sea. Fitness is assessed on an individual basis, considering the stability of your condition, your specific role onboard, and the employer’s requirements.
Asthma: If your asthma is well-controlled with standard inhalers and you have not had recent exacerbations or hospital admissions, you are likely to be passed fit.
Diabetes: Individuals with Type 2 diabetes controlled by diet or oral medication are often deemed fit, provided their blood sugar is stable. Insulin-dependent diabetes requires a more detailed assessment and is subject to strict flag state regulations.
Heart Conditions & High Blood Pressure: These conditions are assessed based on their severity and stability. If your blood pressure is well-controlled with medication and you have no limiting symptoms from your heart condition, you may be passed fit. A recent report from your cardiologist may be required.
Colour Vision & Visual Issues: A certain standard of colour vision is required for deck and engineering roles. If you fail the Ishihara test, further testing may be arranged. If you wear glasses or contact lenses, you must meet the required visual acuity standard *with* your correction.
Joint Replacements (Knee/Hip): Fitness can be certified following a full recovery from surgery, typically after 3-6 months. You must have a good range of motion, stable function, and be able to perform all physical duties of your role without restriction.
Medication: Being on regular medication is not an automatic disqualifier. The condition being treated, the stability of your health, and any side effects of the medication will be considered.
How long is my medical certificate valid for?
A PEME is required prior to work this is different to an ENG 1 medical which is a periodic medical which can be valid for up to 2 years.
For seafarers under the age of 18, the validity is one year. Please note that some employers or flag states may require more frequent medicals.
What do I need to bring to my appointment?
To ensure your appointment runs smoothly, please bring the following with you:

A form of photographic ID (e.g., Passport or Discharge Book).

Your previous seafarer medical certificate, if you have one.

A list of any current medications.

Your glasses or contact lenses if you wear them.

Any relevant medical reports or letters from your GP or specialist concerning existing conditions.

Why is the price different for a PEME medical than it is for an ENG1 medical ?
A PEME medical is required by individual companies and the requirements within the medicals vary and therefore the prices are not set and do vary. An ENG1 medical is a periodic medical and is a statutory requirement required by the MCA for all seafarers to complete and have prior to working as a seafarer and periodically thereafter the price per medical is 115 pound and is set by the MCA itself .
