Every year countless seafarers, marine engineers, deckhands and yacht crew pass through Bristol on their way to work around the world. Before they step aboard any vessel operating under Maritime and Coastguard Agency regulations, they must prove they are medically fit for life at sea. That moment of certification, the ENG1 medical, is more than a box to tick. It is the foundation of safety for crews and passengers.
At Occupational Health Bristol we see the reality of that responsibility every day. An ENG1 is required for anyone who works on tug boats, survey vessels, dredgers, super yachts or cruise ships. If your work places you at sea, this is the assessment that confirms you are physically and mentally capable of carrying out your duties safely. The MCA sets the process and the maximum examination fee of £115, which means every seafarer receives a standardised assessment regardless of where they test
For us as occupational health professionals the duty is clear. Your appointment is led by an MCA approved doctor who checks identification, takes a full history and reviews any previous ENG1 certificate which must be surrendered at the appointment. Examinations include height and weight measurements for BMI, blood pressure, cardiovascular and respiratory checks, abdominal examination and screening for conditions such as hernias and varicose veins. A urine test helps identify early signs of diabetes or kidney issues. In cases where BMI exceeds 35, a fitness test such as a Chester or Queen’s step test may be required.
Even though the assessment is medical, the experience should not feel clinical or intimidating. Our role is to guide seafarers through what is needed, explain what to bring and ensure every candidate feels informed and respected. Because these certificates can last up to two years, and hearing tests up to ten, the impact of getting this right is long lasting.
